SML strives constantly and responsibly to foster quality appliance performance and to improve and increase our appliance offerings. We reserve the right to upgrade quality and design; adjust prices; and/or discontinue items without prior notice. SML provides all appliances as a convenience to our clients, acting as their agent with the manufacturer. We can assume no responsibility for the techniques used (nor their use and/or misuse) by the purchasing doctor, staff or patients.

PAYMENT AND TERMS

All payments may be made via check, Visa, MasterCard, American Express, Discover Card. All balances are due upon receipt of invoice. After 10 days, payments not received are subject to a 1.5% per month charge (amount equal to 18% per year).

APPLIANCE USE AND LIABILITY

SML provides appliances as a convenience to our clients by acting as their agent to the manufacturer. We cannot assume responsibility for the use, misuse or abuse - by purchasing doctor, staff or patients - of said appliances or any techniques used for the seating or placing of bands, crowns, brackets or space maintainers.

APPLIANCE WARRANTY AND CONDITIONS

SML guarantees materials and workmanship on all appliances for 90 days. If an appliance fails within this period, SML will remake the appliance at no charge.

RETURNS

SML guarantees that our appliances will fit your working casts. It is therefore essential that all models be accurate and we-defined upon submission.

If appliance does not fit the returned working cast, SML will fabricate a new appliance on a new working cast at no extra charge. IMPORTANT: All returned appliances must be accompanied by the original working cast and a new accurately defined cast.

Occasonally, original working casts may be slightly damaged during the fabrication process. Should such damage occur, SML will flag patient records, include a note (with appliance) indicating "Damage During Processing," and will remake the appliance at no extra charge. Simply submit new working casts, indicating on the new lab slip that the case is a "broken cast" remake.

PLEASE NOTE: Should you find that your appliance does not fit the patient, yet does fit the working case, you are responsible for taking and submitting new impressions. A new prescription will be required with submission of new casts. SML will then fabricate a new appliance. 

IMPORTANT

Please be advised that many appliances are fabricated with stainless steel (containing nickel and chromium), acrylic and/or nickel titanium. These materials can in some cases trigger an allergic reaction. If such a reaction occurs, advise your patient to consult a physician immediately.
